Transition and Rehabilitation Mental Health Specialist - Get $2 per Hour Monthly Stipend

Get $2 per Hours Worked Monthly Stipend with this Position Until 12/31/24!! 

This position will be responsible for providing transitional case management and skills training to individuals who are moving from crisis services into routine outpatient services to promote stabilization of symptoms and provide support following a crisis episode. The position will use a team-based approach to ensure appropriate communication with the identified treatment team and coordination of care between service providers.  This position will work closely with Crisis Outpatient, PETC and Continuity of Care programs to ensure individuals are supported throughout the continuum of care to prevent deterioration of symptoms. 

This position will be required to provide services to clients in their home and/or community setting as needed as well as in the office.  The schedules must remain flexible and responsive to client needs.  Duties include maintaining current and accurate recovery plans and standardized assessments for each assigned individual,  monitoring service delivery according to the recovery plan and record progress in the electronic health record, work closely with the identified treatment team, and assist in meeting evidence based outcomes identified in the contract . This position requires monthly service performance expectations. Works under limited supervision with considerable latitude for the use of initiative and independent clinical judgment. Requires the ability to adhere to a demanding daily schedule. Applicants must have a TDL and be insurable through Tri-County.

The position is responsible for providing all services using a trauma informed approach: creating a safe environment based on trust, transparency, collaboration, mutuality, and peer support while encouraging empowerment, and acceptance of voice and choice of individuals served, and demonstrating respect for cultural, historical, and gender issues.

Minimum Requirements:

  • Requires a Bachelor's Degree in a Behavioral Science field. Applicants must qualify as a Qualified Mental Health Professional (QMHP) in State of Texas.
  • 2 years experience in human service field preferred.
  • Bilingual preferred.
  • Applicants must have a valid drivers license and be insurable through Tri-County.

Payscale:

Offenses that are not an absolute bar to employment but may be considered a contraindication will also be considered during the pre-employment process. Tri-County will also complete a driver's license check. Applicants who have lived out of state for two or more years from the date of submission of this application must have a Federal background check by fingerprinting through the FBI if they are selected for a position.
