Here's what you'll do:

  • Overlooks the care and provides routine Case Management duties for a caseload of many Adult MH Individuals
  • Administers Behavioral Health assessments
  • Maintain current and accurate recovery plans and assessments for each individual
  • Monitor service delivery according to recovery plans and record progress in progress notes
  • Work closely with the identified recovery team
  • Authorize ongoing care of individuals
  • Assist in meeting evidence-based outcomes as per contract
  • Coordinate care and provide community referrals
  • Ability to work under limited supervision with available support from supervisor and team
  • Adheres to fast paced daily schedule
  • Meet monthly service performance expectations.
  • Meet clients in an office setting
  • Provide Trauma-Informed care and interventions

The position is responsible for providing all services using a trauma informed approach: creating a safe environment based on trust, transparency, collaboration, mutuality, and peer support while encouraging empowerment, and acceptance of voice and choice of individuals served, and demonstrating respect for cultural, historical, and gender issues.

Here's what you'll need:

  • Requires Bachelor's Degree in a Behavioral Science field.
  • Applicants must qualify as a Qualified Mental Health Position (QMHP) as defined by Department of State Health Services (DSHS).
  • Bilingual in English and Spanish preferred, will be paid a bilingual stipend.
  • Applicants must have a valid Driver's License and be insurable through Tri-County.

Payscale:

$44,938/yr**

Offenses that are not an absolute bar to employment but may be considered a contraindication will also be considered during the pre-employment process. Tri-County will also complete a driver's license check. All applicants, employees, contractors and volunteers will be checked prior to hire and monthly through the Texas Health and Human Services Commission Office of the Inspector General (OIG) List of Excluded Individuals. All applicants are required to successfully complete a drug screening and a physical agility test prior to new hire orientation.

Applicants who are selected for a position in the Criminal Justice/Jail Diversion Department, TCOOMMI, and those who provide services to TCOOMMI, i.e., psychiatrists, nurses, etc., must have a Federal background check by fingerprinting through the FBI upon initial hire and annually thereafter.

Applicants who have lived out of state for two or more years from the date of submission of this application must have a Federal background check by fingerprinting through the FBI if they are selected for a position.
