Care Coordination Specialist

The Care Coordination Specialist is responsible for coordinating care for individuals with serious mental illness, including those with co-occurring substance use disorder and medical needs. 

What you will do:

  • Maintaining a shared caseload of individuals in need of assistance with care coordination activities.
  • Providing evidence-based treatment to improve health behaviors.
  • Connecting clients to community resources, providing education on health conditions, following up with providers, and guiding clients across healthcare settings.
  • Serving as a liaison between internal and external treatment partners to maintain appropriate communication, minimize duplication of services, promote successful outcomes, and increase integration of care.
  • Identifying and developing relationships with external providers and referral sources to facilitate seamless connections across programs.
  • Completing consents for release of information with clients to allow for sharing of clinical records and relevant client information between treatment partners.
  • Developing, directing, and providing other treatment and support as clinically indicated.

Works under minimal supervision with extensive latitude for the use of initiative and independent judgment. This position will function under the supervision of the Project Director and within the scope of practice. Adheres to professional practice within scope of QMHP certification, quality assurance standards, and all applicable policy and procedures. This position works closely with community social service providers, individuals served and their families, MDs, APNs, case managers, and other nursing staff to provide a team-focused atmosphere. This position is based out of our Conroe office, with services largely provided in 
the community.

Position Requirements:

  • Qualifies as a Qualified Mental Health Professional (QMHP) in Texas, including minimum of Bachelor's Degree in behavioral health field
  • Bilingual in English and Spanish preferred
  • Valid driver's license and insurable through Tri-County

Payscale: 

$48,776/yr

Offenses that are not an absolute bar to employment but may be considered a contraindication will also be considered during the pre-employment process. Tri-County will also complete a driver's license check. All applicants, employees, contractors and volunteers will be checked prior to hire and monthly through the Texas Health and Human Services Commission Office of the Inspector General (OIG) List of Excluded Individuals. All applicants are required to successfully complete a drug screening and a physical agility test prior to new hire orientation.

Applicants who are selected for a position in the Criminal Justice/Jail Diversion Department, TCOOMMI, and those who provide services to TCOOMMI, i.e., psychiatrists, nurses, etc., must have a Federal background check by fingerprinting through the FBI upon initial hire and annually thereafter.

Applicants who have lived out of state for two or more years from the date of submission of this application must have a Federal background check by fingerprinting through the FBI if they are selected for a position.
